Questions to consider
- Is the survey about one process, or several? Send a separate link per process so answers don't blur together.
- Will people answer honestly if their manager can see the results? Decide who reads them and say so up front.
- Which fixes in the ranking list are actually in your power to make?
- Do you want to hear from people upstream and downstream of the process, not only the ones inside it?
How to use the responses
Group answers by role: the people doing the work see different problems from the people handing work in or relying on the result. Workarounds are the most useful answers here, because each one marks a gap someone has already solved for themselves and could be made standard. Take the top-ranked fix and the most common time sink, pick one to change first, and tell everyone what you chose and why.
How to customize and share it
- 1Put the process name in the greeting, or add it as a hidden field and remove the first question.
