Questions to consider
- Will you send this once at the end, or at the start, middle and end as well? The stage question lets one form cover all three.
- What did the program promise participants? Make sure the rows in the grid cover each of those things.
- How will you reach people who left early, since they're the least likely to open an email from you?
- Do funders or partners need any of these answers in a report? Add the measures they ask for now.
How to use the responses
Read the answers from people who left early first, because they point at the problems that cost you participants. Compare the usefulness grid by stage to see whether a part of the program helps at the start but fades later. Use the barriers question to separate what you can fix, like session times, from personal circumstances, and share a short "you said, we did" note with the next cohort.
How to customize and share it
- 1Rename the grid rows to the actual parts of your program, such as workshops, mentoring or site visits.
