Questions to consider
- Which parts of the session are you able to change, and does the grid ask about those?
- Should the session name be filled in for people through the link, rather than typed?

- Are you measuring reaction to the session here, and learning or behaviour change somewhere else?
How to use the responses
Read the change requests first and group them by part of the session, then check them against the grid: if many people disagree that there was time to practise, that's a design fix rather than a trainer fix. Compare pace and level answers across groups before changing the content, since a split between too basic and too advanced usually means the audience needs sorting, not the material. Share the results with the trainer alongside the quotes about what worked best.

Should training feedback be anonymous?
It usually gets more honest answers. This template doesn't ask for a name, and you can add one if you need to follow up with individuals.
