How to use the responses
Sort answers by visit goal first, then by whether people found what they came for. A cluster of "No" answers from people comparing prices points at a specific page, and the written answers about where they looked tell you where to put the missing link. Fix the most common gap, then compare the same question a month later to see if it moved.
How to customize and share it

Frequently asked questions
What questions should a website questionnaire include?
Ask why the person came, whether they managed to do it, and where they got stuck. Those three answers are more useful than a general rating of the design.
When should I show a website questionnaire to visitors?
After they have had time to try something, such as at the end of a visit or after a key page. Asking on arrival gets opinions about a site they have not used yet.
Is a website questionnaire the same as a website design brief?
No. A brief collects what a client wants built. This questionnaire asks real visitors how an existing site works for them.
Can visitors answer without leaving an email?
Yes. The email question at the end is optional, so people can give feedback anonymously if they prefer.
