- Is this survey about one course, a whole department, or the institution as a whole?

How to use the responses
Pass contact requests to student support the day they arrive; those students have told you they are struggling. For the rest, compare the teaching grid by course and year, since feedback timing and workload problems usually sit with a specific module or cohort. Report back to students on two or three changes you made, because that is what gets them to answer next time.
How to customize and share it

What questions are in a student satisfaction survey?
Most cover teaching quality, feedback and assessment, learning resources, workload, support services and an overall rating. An open question about one change gives you the detail behind the scores.
When should a student satisfaction survey be run?
Near the end of a term or semester, once students have had assessments back but before exams take over. Running it at the same point each year makes the results comparable.
Can student satisfaction surveys be anonymous?
Yes. This template never asks for a name. It only asks for an email if a student chooses to be contacted by support.
How do I get more students to respond?
Keep it short, send it through channels students already check, and show them what changed after the last survey. A chat format that asks one question at a time also works well on phones.
