How to use the responses
Split the answers by how long people have used the product, because first-week impressions and six-month verdicts tell different stories. Reply to every buyer who asked for contact before you analyse anything else. Then look at which row of the rating grid drags the score down, and read the written answers from disappointed buyers to find out why.
How to customize and share it

After the buyer has used the product enough to judge it. For something used daily that might be a couple of weeks; for something seasonal, longer.
What questions should a product satisfaction survey include?
An overall rating, ratings for the qualities that matter, whether it met expectations, what they like or what fell short, and how likely they are to recommend it.
How is product satisfaction different from customer satisfaction?
Product satisfaction is about the item itself. Customer satisfaction also covers delivery, support and the buying experience, so keep those questions separate if you want to know which one to fix.
Can I follow up with unhappy buyers?
Yes. Buyers who say the product was worse than expected are asked if they want to be contacted, and only those who say yes are asked for an email.
