How to use the responses
Use the answers as a starting point for conversation, never as a label or a reason to make a decision about someone. Read each person's open answers next to their choices, since that is where the useful detail sits. Where someone said pressure overwhelms them, follow up privately on what they said would help. For a team session, look across the grid to spot where people differ, such as routine versus variety, and talk about how to work with that.

Is this personality questionnaire a validated assessment?
No. It is a set of reflection questions for conversation. Do not use it to hire, promote or rate people.
What questions go in a personality questionnaire for work?
Ask about everyday situations: how people start a task, how they recover energy, how they like feedback and how they react under pressure. Concrete situations are easier to answer honestly than abstract traits.
Should answers be anonymous?
For team building, people usually want their name attached so colleagues can learn about them. Let each person choose who sees their answers, as this template does.
