Questions to consider
- Will you use one link per session, or one form for many sessions with the name asked or prefilled?
- If several trainers run the same course, do you need the trainer's name as its own question?
- Does the setup row matter for your format, or should it name the venue or online platform you used?
- Who needs the results: the trainer, the course owner or the participants' managers?
How to use the responses
Sort responses by overall rating and read the "what let it down" answers from the lower scores first; the most common cause is usually the first thing to fix. Use the satisfaction grid to spot a weak area that even happy participants rated poorly, such as materials or joining instructions. Compare in-person and online answers, since setup problems often hit one group only. Share the "best part" answers and the repeat-trainer question with the trainer so they know what to keep.
How to customize and share it

Satisfaction is how people felt about the session. Effectiveness is whether they learned and use it. This template adds a confidence question as an early signal, but a follow-up weeks later tells you more.
Can I use one survey for several training sessions?
Yes. The first question asks which session someone attended, so you can filter or compare sessions when you export the answers to CSV.
