Questions to consider for your event volunteer form

  • What are your real shifts, with times? Replace the shift options so volunteers know what they're agreeing to.
  • Which roles need a particular skill, such as first aid or a driving licence?
  • Do you accept volunteers under 18, and what does your organisation need from a parent or guardian?
  • Does your volunteer code need to cover anything specific to your venue, such as alcohol or safeguarding?

How to use the responses

Export the responses to CSV and build the rota from the shifts column first, then fill each role from the rankings, giving people their first or second choice where you can. Put first aiders and drivers where they are needed before anything else. Confirm under-18 volunteers with their parent or guardian before sending shifts. Keep the emergency contacts in a printed list with the coordinator on the day.

How to customize and share this template

  1. 1Replace the shift and role options with your event's real schedule and jobs.
  2. 2Edit the volunteer code to your organisation's wording, or remove it if you use a separate agreement.
  3. 3Share the link with your community, or embed it on your event page, well before the rota is due.

Frequently asked questions

What should an event volunteer form include?

Contact details, which shifts someone can cover, which roles they prefer, relevant skills, an emergency contact and agreement to your volunteer expectations.

How far in advance should I recruit event volunteers?

Early enough to build a rota, send a briefing and replace anyone who drops out. For a large event that often means several weeks ahead.

Can volunteers under 18 use this form?

Yes. They are asked for a parent or guardian contact before they continue, so you can check with that person before confirming shifts.

How do I turn the answers into a volunteer rota?

Export the responses to CSV, sort by shift, and assign roles using each person's ranking. The skills answers tell you who to place first.
