Yoga registration form template
For studios and independent teachers. It places each student by class and experience, asks when they can come, and only asks about injuries in detail when there's something to tell. Anyone booking prenatal yoga is asked how far along they are and whether their midwife or doctor is happy for them to exercise.

Questions to consider
- Which classes do you run, and should any of them ask their own follow-up questions like prenatal does?
- Do you offer online classes, or only in the studio?
- What should your participation agreement say, and has anyone checked the wording for your area?
- Who reads the health notes, and how will you keep them private?
How to use the responses
Before each class, check the list for health notes and prenatal students so the teacher can plan modifications, and contact any prenatal student who answered 'Not yet' about clearance before their first session. Point complete beginners who picked a flow class toward a beginners' session first. Use the preferred times across all sign-ups to decide when a new class would fill, and export the list to CSV for your register.
How to customize and share it
- 1Replace the class list with your timetable, and route any class that needs extra questions to its own follow-up.
- 2Rewrite the participation agreement in your own words, keeping it short and clear.
- 3Share the link from your website, social profiles and the studio front desk, or embed it on your class page.
Frequently asked questions
What should a yoga registration form ask?
Contact details, the class, experience level, preferred times, any injuries or conditions, an emergency contact and a participation agreement.
Should a yoga studio ask about injuries?
Yes, so the teacher can offer modifications. This form asks one yes or no question first and only asks for details when there's something to share.
Is it safe to take a yoga class while pregnant?
Many people practise through pregnancy, but it depends on the person. That's why the prenatal path asks whether a midwife or doctor has said it's fine to exercise.
Can I use this form for online yoga classes?
Yes. Students choose studio or online, and you can send the joining link in your follow-up.
