Questions to consider for your employee referral form

  • Which roles are open right now, and should the list update as they change?
  • Do you pay a referral bonus, and what does the referrer need to know about it?
  • Who follows up on referrals where the candidate hasn't been asked yet?
  • Should referrals for no specific role go into a general talent pool?

How to use the responses

Contact only the candidates whose referrers said they know about it, and use the approach the referrer suggested. For the ones not yet asked, send the referrer a reminder after a few days. Read the vouching score together with how they know the candidate: a strong vouch from someone who worked with them directly is worth fast-tracking. Tell referrers what happened either way, because silence is what stops people referring again.

How to customize and share this template

  1. 1Replace the role list with your open teams or current job titles.
  2. 2Add a line to the greeting about your referral bonus or policy, if you have one.
  3. 3Share the link in your internal chat and staff handbook, and check new referrals in the dashboard each week.

Frequently asked questions

What should an employee referral form include?

The referrer's details, the candidate's name and contact, the role, how they know each other and why they would be a good fit.

Should the candidate know they've been referred?

Yes. Contacting someone who didn't expect it is awkward for everyone. This form asks, and only collects contact details once the candidate is on board.

How do you make an employee referral program work?

Keep referring quick, follow up with every referrer, and tell them the outcome. People who never hear back tend to stop referring, whatever the bonus.
