How to use the responses
Review new requests on a fixed day each week rather than as they arrive, so priorities are set together instead of by whoever asked loudest. Sort by due date and by whether the date is fixed: a launch-tied deadline outranks a preferred one. Reply to every requester with a decision, and export the responses to CSV each quarter to see which teams ask most and which request types take longest.

What should a marketing request form include?
Who is asking, what they need, the audience, the goal and the deadline. Asking whether the deadline is fixed saves a lot of back and forth when two requests clash.
How far in advance should people submit marketing requests?
It depends on the work. Many teams ask for a few days for a social post and several weeks for a full campaign; say your own lead times in the greeting.
Can different request types ask different questions?
Yes. This form already branches: a campaign request is asked about goals and channels, while a content or design request is asked where the work will be used.
