Information request form template
Asks what the person needs, what it is for and how they want to receive it. Anyone asking for records about themselves is asked for a reference and proof of identity before the rest, and a request for a printed copy asks for a postal address, so nobody is chased for details afterwards.

Questions to consider
- Which kinds of information can you share, and which should the form say you can't?
- Do personal record requests need an identity check in your setting, and what counts as proof?
- Are you bound by a response deadline for some requests, and should the ending mention it?
- Which delivery methods can your team really offer?
How to use the responses
Log each request with the date it arrived, because many information requests carry a response deadline set by law or policy. Check the identity proof before releasing any personal records, and delete uploaded ID once it has been checked if you don't need to keep it. Group requests by topic every few months: questions that keep coming back are usually a sign the answer should be published on your website.
How to customize and share it
- 1Edit the topic list to the kinds of information people actually ask your organisation for.
- 2Change the identity check wording to match what your organisation accepts as proof.
- 3Link the form from your contact, privacy or help page so requests arrive in one place.
