Reseller application form template
Built for a partner manager who gets more reseller requests than they can call back. It asks the questions that separate a real sales channel from a curious buyer, stops early for businesses that are not registered yet, and only asks about competing brands when the applicant already sells some.

Questions to consider
- Do you need resellers to be registered businesses, or can individuals and side businesses apply?
- Are there regions or channels you already cover and would not want a new reseller competing in?
- Would a reseller who carries a direct competitor be a problem, or a sign they know the market?
- What minimum monthly volume makes a reseller worth the onboarding effort for your team?
How to use the responses
Sort applications by expected volume and business type first, then read the target market and why-join answers for the ones that fit. The similar-brands answer is often the most telling: a reseller already moving a comparable product has customers waiting. Filter the dashboard by country to spot overlap with partners you already have, and export to CSV if a regional manager needs their own shortlist.
How to customize and share it
- 1Swap the channel and support options for the ones your program actually offers, and remove the registration gate if you accept individual sellers.
- 2Rewrite the volume ranges in the units you sell, such as licences, cases or subscriptions.
- 3Link the form from your partners page or embed it there, and send approved applicants your onboarding pack by email.
Frequently asked questions
What should a reseller application form ask?
Contact details, the business name and registration, how and where they sell, who their customers are, and why they want your products. Expected volume helps you prioritise which applications to answer first.
How do I screen out applicants who are not a fit?
Ask the deal-breaker questions early. This form checks registration fourth and ends politely for anyone not registered, so they never fill in the rest.
Should I ask resellers about competing brands?
Yes. A reseller who already sells a similar product knows the buyers, and knowing which brands they carry tells you how you would sit on their shelf or site.
Can I use this as a distributor or dealer application instead?
Yes. Copy it with Use this template, rename it, and adjust the business type and volume options. Every question can be edited.
