Questions to consider for your quote request form
- What are the three or four kinds of job you quote most often?
- For each kind, which one or two answers change the price the most?
- Is there a budget below which you'd rather decline politely?
- How quickly can you promise a reply, and does the ending say so?
How to use the responses
Group requests by job type and price each group from its own answers: scale and design for new builds, the current link and reasons for redesigns, hours and response time for support. When the budget and the scope don't match, reply with what the budget does buy rather than a flat no. Keep the ending's reply promise, or change it to one you can keep.
How to customize and share this template

What should a quote request form include?
Contact details, the type of job, the details that affect price for that job, a description, a budget range and a deadline. Anything that doesn't change your price can wait until the call.
How is a quote request different from a contact form?
A contact form starts a conversation. A quote request collects enough scope that your first reply can be a price instead of a list of questions.
Can the form ask different questions for different jobs?
