How to use the responses
Sort by timeline and budget together: approved budget with a date inside three months is your call list for today. Before each call, read the goal answer and the ranking so you can open with what they care about most. When the person filling in the form does not make the final decision, ask on the call how to bring the decision maker in. Put the explorers on a slower email track and check back each quarter.

What questions should a lead qualification form ask?
Goal, company size, current solution, who decides, what the buying process involves, budget and timing. Together they tell you whether and how fast to follow up.
Is this based on BANT?
It covers budget, authority, need and timeline, and adds two things BANT leaves out: how they solve the problem today and what has to happen before they can buy.
Won't asking about budget put people off?
Asking whether budget exists, rather than how much, is easier to answer. The three choices let people say it's not set yet without feeling judged.
What happens to leads who are just exploring?
