How to use the responses
Treat each response as the first version of the customer's record. Copy it into whatever system you bill or deliver from, or export the responses to CSV and import them in a batch. Check the marketing answer before adding anyone to a mailing list, and honour the preferred contact method and time from the first call onwards. The notes field is where access details and special requests hide, so make sure it reaches whoever serves the customer.
How to customize and share it
